Community Led Integrated Health Care

Community Led Integrated Health Care (CLIHC) is a new way to health and wellbeing for children aged 0-12.
Our CLIHC clinic is a ‘one stop shop’ where families can access health, wellbeing and allied health providers who offer a range of services your child needs. Our Care Coordinators will help create a plan for you to access these services in a way that works for your family.
CLIHC is a free program provided by Berry Street and Latrobe Community Health Service, in the Latrobe Valley.
What does this service include?
- A one stop shop for all your children’s health and wellbeing needs
- Care Coordinators to help you throughout your family’s journey
- Access to health and wellbeing professionals
- Working as a team to help you achieve your goals.
How does it work?
Once a referral has been received, our team will work with families to understand their goals and needs.
A care plan will be created to link families in with other support and health services.
As part of this care plan, families are supported with things like a list of upcoming appointments, a folder of any relevant reports and letters, and anything else they need to access care.

Eligibility
- Resident of Latrobe Valley
- Families who would benefit from supported access to health and wellbeing
- Family has a child aged 0-12 years
- Child or family is experiencing vulnerability.
